Assoc. Prof Dr Ng Ruey Terng is a Paediatric Gastroenterologist and Hepatologist practising at University of Malaya Medical Centre, Kuala Lumpur. He is one of the trainer for paediatric endoscopy in the country.


Acute Hepatitis of Unknown Origin in children-an update

There has been a sudden surged of acute hepatitis among children aged mainly below 5 since early 2022. The initial alarm came from Scotland and Alabama where children of below 5 years who was previously healthy presented with acute hepatitis of unknown origin and with significant morbidity and mortality, and it quickly spread to Europe and other part of the world. Human adenovirus (HAdV, predominant type 41) was isolated in more than 50% of the cases, and a small cohort of not more than 20% with evidence of SARS-CoV-2 infection. There was no liver histological evidence of direct infection of HAdV otherwise. About 5% of the patients worldwide needed a liver transplantation, and 2% has died. WHO has rated the risk of the disease as moderate and encouraged member regions to actively report and investigate all the newly diagnosed cases. Attention has been focuses on the hypothesis of post SARS-CoV-2 immune dysregulation as well as immunological naïve of children secondary to preventive strategies during pandemic.
